Re:                   Enhanced Disability Management Program

Please be advised that as of April 1, 2017, Disability Management Institute (DMI) ceased to be the service provider for the Enhanced Disability Management Program (EDMP).    Instead,  Bayshore Home Health has retained an Independent Disability Management Advisor (DMA), Ms. Andrea Duffels, who is now providing assistance to Bayshore Community Health Care employees enrolled in EDMP.

Please use this contact information if you are self-referring into the Program, or if you have been referred into the Program by Bayshore's Disability Management Department.

Sheryl Quail remains your Union EDMP Advocate.  She may still be reached at 604-828-2815.  Her hours of work are Tuesdays and Thursdays, 8:00 a.m. to 4:00 p.m.  She will provide EDMP Union support to Bayshore Community Health Care Workers who have been referred to and who have enrolled into the Program.
