Changes made to distribution of disability benefit forms for public service workers

As part of an effort to reduce it’s environmental footprint, the BC government has initiated changes to the distribution of Long Term Disability (LTD) application forms. In place of the previous practice, which automatically mailed forms to members, the employer is now making the forms available in electronic PDF form online:

Physician's Medical Form – For LTD Application forms to be completed by your physician click here.

The forms can be printed and taken to your physician; or they can be completed online and submitted electronically by your physician.

If you require a printed copy – Contact Disability Benefits Administration (DBA) at: 1-877-387-0457, or in Victoria at 250-387-0457.

As in the past, members will still receive a letter at approximately the three month mark of STIIP which will provide this information and encourage members to apply for LTD, to ensure there is no impact to their benefits if they remain ill or injured and require LTD upon expiry of STIIP.
