Following discussion of the issue at a recent membership meeting at The Crest Hotel, your bargaining committee is seeking additional input from you on the subject of extended health benefits to help inform upcoming contract negotiations with your employers.

A hard copy of the mini-survey is being mailed to you in case you would prefer to complete it by hand. Alternately, you may print the attached PDF yourself and complete by hand. Completed hard copy mini-surveys may be returned by:
